The Nurse Practitioner performs physical examinations, formulates patient care plans, and prescribes treatments in collaboration with physicians. They monitor patient progress and work closely with the interdisciplinary hospice team to provide high-quality care.
Candidates must have completed an accredited Nurse Practitioner program and hold a valid state license in good standing. The role requires the ability to perform physical tasks like lifting patients and maintaining reliable transportation for travel across assigned territories.

Records physical findings, and formulates plan and prognosis, based on patient's condition. Discusses case with physician and other health professionals to prepare comprehensive patient care plan. Prescribes or recommends drugs or other forms of treatment. Monitors and maintains progress of patients and reports progress to supervising physician. Has the ability to work collaboratively with hospice employees and volunteers as part of an IDT Territories Include: Hanncock, Henry, Madison, Delware, Shelby Rush, Fayette
While performing the duties of this position, the employee is regularly required to assist in lifting and transferring patients as required. Specific vision abilities required by this job include: close vision, distance vision and ability to adjust focus. Requires completion of accredited Nurse Practitioner program. State licensure in good standing Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance
