The Hospice RN Case Manager coordinates and administers skilled nursing care to terminally ill patients in home or facility settings. They are responsible for managing a patient caseload, collaborating with the interdisciplinary team, and ensuring the delivery of high-quality end-of-life care.
Candidates must be a graduate of an accredited nursing school and hold a current Registered Nurse license. A minimum of one year of clinical nursing experience is required, along with the ability to manage multiple priorities and travel to patient locations.

Pay Range $80,000—$85,000 USD
Employment is contingent upon the successful completion of a background check. Screening is completed by a third-party administrator, the Michigan Long-Term Care Partnership, and is performed in compliance with the Fair Credit Report Act.
