The TCOOMMI Case Manager provides case management, skills training, and psychosocial rehabilitation to adult mental health parolees and probationers. They are responsible for treatment planning, crisis intervention, and coordinating services to support community reintegration and reduce recidivism.
Candidates must hold a Bachelor's degree in Psychology or a Human Services field and be eligible for certification as a Qualified Mental Health Professional. A valid Texas driver's license and an acceptable driving record are required for this role.
This position will also be responsible to provide continuity of care (COC) services, transitional case management (TCM) services, and/or intensive case management (ICM) services to people on probation and parolees released from the state prison system, ISF facilities and SAFP facilities. This position will be responsible for providing case management, skills training, psychosocial rehab and crisis services to the caseload with the goal of maintaining their mental stability to reduce recidivism back to state prison and other corrections facilities. This position will work to transition the consumers into lower levels of care and to reintegrate the individual back into the community.
Must be able to work independently, while maintaining professionalism with clients and community stakeholders.
This position is directly supervised by the MH Program Administrator/ TCOOMMI Program Director.
Hours for this position are Monday - Friday, 8am-5pm, flexible.
This is a Full-Time position. Benefits are available.
Will be eligible for a one- and two-year increase, depending on satisfactory performance.
