Salary
$30 / HOUR
Oversee care for high-risk patients transitioning from healthcare facilities and coordinate with primary care provider teams. Handle clinical tasks, manage patient data, and manage incoming patient communications and scheduling.
Must be a graduate of an accredited LPN program with an active Pennsylvania Practical Nursing license and BLS certification. Experience with Medent EHR and working within the EOS model is preferred.
