The Licensed Practical Nurse will provide direct nursing care to individuals with developmental and physical disabilities by visiting group homes and conducting clinical assessments. Responsibilities include medication delivery, updating medical records, educating staff on care plans, and coordinating with healthcare professionals.
Candidates must have graduated from an accredited practical or vocational nursing program and hold a current LPN license in Pennsylvania. A valid driver's license and reliable transportation are required, with at least one year of experience preferred.

As a Licensed Practical Nurse (Group Home Visiting Nurse) at AVS Merakey, you'll provide direct nursing care to individuals with developmental and physical disabilities. Your responsibilities will include traveling to our group homes, clinical assessments, monitoring acuity, teaching staff on residents care plan and medical needs, delivering medications to group homes, updating MAR and Treatment logs.
