The mental health worker provides psychosocial assessments, intensive case management, and treatment planning to support patient wellness. They also facilitate community resource linkages and provide crisis intervention for patients and their families.
Candidates must hold a Master's degree in social work, counseling, or a related human services field and possess a valid LMSW, LCSW-C, LGPC, or LCPC license. A minimum of two years of experience in a social work or counseling setting is required.
Under direct supervision of HARP director, The HARP mental health worker would provide psychosocial assessments, intensive case management services, treatment planning and consultation to HARP patients. The goal is to assist patients and the health care team in coping with patient’s illness, diagnosis, treatment, and/or life situation. The mental health will also link patients with available resources in the community to help then maintain optimum wellness; and other duties as assigned.
