• Shift: DayJoin our growing team! CareOne Home Health is locally owned and growing company seeking an Admissions RN. This position is full time. Our goal is to continually serve our community by providing high-quality, compassionate care in the intimate setting of our patients' homes. We are currently looking for an Admissions RN to serve patients in the central valley. Home health experience is required. Benefits Competitive wage One to one patient care Medical benefits 401k matching Continued education Supportive and friendly work environment Skills Minimum one year nursing experience required One year Home Health experience required Palliative experience strongly desired Knowledge of nursing care processes and medication guidelines/side-effects Committed to safety precautions and high ethical standards Proficient in reading/writing English, bilingual a plus (Spanish preferred) Sound judgement and attention to detail Excellent communication skills and a compassionate nature Cool-tempered and patient Emotional/mental stability and stamina Additional Requirements CA Registered Nurse license required 1 year nursing experience required Home Health experience required Current CPR certification required Must have reliable transportation and auto insurance Responsibilities Make initial visits to patients to complete comprehensive assessment and determine home health needs Collaborate with physicians and the Interdisciplinary Team to formulate and review care plans Make frequent visits to the patient to evaluate their condition Provide support and comfort according to individual patient’s needs Work with caregivers to plan and execute excellent patient care Notify a doctor for a patient’s condition when deemed necessary Assist patients and their families with paperwork Maintain accurate records Qualifications
