The Hospice Social Worker provides medical social services to clients in home and facility environments to meet individualized outcomes. Responsibilities include developing plans of care, counseling families on emotional responses to illness, and coordinating community resources.
Candidates must have a graduate degree in social work and be a Licensed Masters Level Social Worker (LMSW) in South Carolina. A minimum of two years of social work experience in a healthcare setting is required.
Participates in development and re-evaluation of POC for clients needing MSW. Instructs and counsels the client and family in treating and coping with the social and emotional responses to illness. Participates in discharge planning. Participates in care coordination activities and act as resource to other team members in identification and resolution of client needs. Utilizes and connects patients and their families with community resources. QualificationsQualifications: Graduate degree in social work or related field LMSW or independently licensed LISW-CP in South Carolina Preferred: Independently licensed LISW-CP in South Carolina with the ability to provide clinical supervision. Excellent organizational, communication, and time management skills. Minimum of two years social work experience in a healthcare setting. Excellent interpersonal and analytical skills.
