The LPN will deliver hands-on nursing care, administer medications, and monitor resident conditions to ensure high-quality care. They will also collaborate with the interdisciplinary team, supervise medication aides, and maintain accurate clinical documentation.
Candidates must hold an active LPN license and current CPR certification. Strong clinical judgment, communication skills, and a compassionate, resident-first mindset are required for this role.

What You’ll Do Deliver hands-on nursing care to residents Administer medications and treatments safely and accurately Monitor resident conditions and report changes to the care team Collaborate with RNs and interdisciplinary team members Support care planning and resident assessments Supervise Medication Aides/Techs Assist with admissions, discharges, and emergencies Maintain accurate, timely documentation
