Shift: Split ShiftAlternatives, Inc. is currently hiring a full-time Case Manager for the Franklin House program located in Somerset, NJ. The Franklin House Transitional Housing Program provides housing for homeless women and their children in a home like atmosphere. The families are each provided a private and comfortable living space in which they reside for approximately 12 months. During this period of time, the women are involved in educational programs, build work-readiness skills, secure employment, gain parenting, financial and other daily living skills. Staff are on-site 24/7 to provide services and support as needed.

Responsibilities of a
Case
Manager: Assist clients in developing goals oriented towards gaining self-sufficiency, parenting skills, finding permanent housing, etc. Provide on-site services and support, including provision of direct service to clients in areas such as money management, daily living skills, psycho-education, work readiness skills, etc., in order to be able to transition into permanent housing Provide transportation as needed to assist with child care, grocery shopping and other needs in the community. Support families personal outcomes to enhance quality of life Utilize an electronic health record (EHR) system to write clear and concise progress notes

Training: All employees are required to complete 10-15 trainings upon hire and on an ongoing basis. Trainings are a combination of in person, which require staff to commute to the main office in Raritan NJ, and remote, using Go To Meeting. All trainings are paid. BENEFITS
