The Care Coordinator provides social services and care management to non-complex members, including assessments, referrals, and health risk screenings. They collaborate with healthcare providers and maintain timely documentation of all care management activities.
Candidates must hold a Bachelor’s degree in social work or a related healthcare field and possess at least one year of experience in healthcare and social services. Additionally, a Basic Life Support certification is required within 30 days of hire.
Targeted salary $20.00 per hour, based on experience

HSC Tulsa is hiring a Social Work Care Manager through OU's PEAK Temporary Staffing Service. This position is designed to be temp to permanent hire.
The Care Coordinator is responsible for providing care management services to non-complex members.
It is a hybrid position with 2 days in the office required, with Wednesday being one of the required days.
In Person only for first 3 or 4 weeks of Training AND
In Office Tuesday, Wednesday and Thursday for 1st 90 days or until successfully complete the Competency and Experience Check List
After that, can be at home 3 days and In Office Wednesdays and their office day of choice
• Basic Life Support Certification within 30 days of the hire date in this position.
• Licensed Social Work Associate (LSWA).
