The Case Manager facilitates the intake process, conducts biopsychosocial assessments, and develops personalized treatment plans for clients in a detox and stabilization program. They provide ongoing counseling, coordinate aftercare resources, and maintain accurate clinical documentation in accordance with agency guidelines.
Candidates must hold an associate degree with certifications or a combination of relevant experience and education, with a bachelor's degree preferred. Proficiency in both Spanish and English is required, along with a compassionate approach to working with individuals in substance use recovery.
About the program Casa’s Conexiones Individualized Treatment Services is an integrated Acute Treatment Services/Clinical Stabilization Services (ATS-Detox/CSS) program that supports individuals from medically monitored withdrawal management through clinical stabilization and works to transition patients to ongoing care in the community. This model provides increased support for individuals with co-occurring mental health and substance use disorders who have had unsuccessful previous treatment. Staff take a personalized approach to meet each patient’s needs and help them develop a treatment plan so they can thrive in the community. Our goal is to stabilize and support patients as they move forward in their recovery.
Conexiones is a 32-bed inpatient facility located on the campus of Tewksbury Hospital. Our care plans integrate the CSS and ATS-Detox services tailored to individual needs.
The Clinical Stabilization Services Program (CSS) includes education and counseling regarding the nature of addiction, relapse prevention, and aftercare. The Case Manager helps the client during this post-detox process to identify their goals, needs, and resources during this time-limited stay. From the assessment, the Case Manager and the client formulate a plan together to meet those goals. The eventual goal along the continuum can range from returning home to further residential placement.
