Job Summary We are seeking a compassionate and dedicated Wound care Nurse Practitioner (NP) to join our dynamic team.
In this role, you’ll play a vital part in delivering comprehensive Wound care care to patients, working closely with an interdisciplinary team in both long-term care and acute settings.

Responsibilities
Provide specialized wound care consults in nursing facilities
Evaluate and manage pressure ulcers, diabetic ulcers, surgical wounds, and vascular wounds
Perform wound assessments and staging
Conduct bedside procedures such as debridement and advanced wound treatments
Develop wound care treatment plans including dressings, offloading, and infection management
Collaborate with facility staff to prevent wound development and recurrence
Educate nursing staff and caregivers on wound prevention strategies
Document progress and treatment outcomes in EMRs
Qualifications
Minimum of 6 months of experience in a long-term care or skilled nursing facility (Hospital/outpatient office experience will also be considered)
Active and unrestricted NP license
DEA license required
CPR certification (BCLS/BLS)
Master’s degree in Nursing (NP applicants)
Graduation from an accredited NP program
Preferred: Experience in Wound care (training available if not)
Preferred: Familiarity with EMR systems (e.g., Sigma, Visual, PCC – training provided)
