- Baptist Health is looking for a Home Infusion RN Hospital Liaison to join our Home Infusion team at Baptist Health Louisville! Monday
- Friday from 9am -5:30pm. On call rotating weekends for new referrals leaving the hospital. Advocate for patients and families, ensuring their home infusion needs are properly identified and met. Educate patients and their families on infusion procedures, medication administration, and how to manage potential side effects. Provide guidance on maintaining a sterile environment and proper use of infusion equipment. Educate patients and their families on managing illnesses or injuries, including post-discharge care and chronic condition management. Maintain accurate, timely, and detailed documentation of all patient care activities in compliance with department and regulatory standards. Regular and reliable attendance is an essential function of this role. Healthcare workers must be present and punctual for all scheduled shifts to ensure the delivery of high-quality patient care. Provide education to hospital staff about home infusion services. Collaborate with the healthcare team, including physicians, pharmacists, and case managers, to develop and implement individualized care plans. Serve as the primary contact between home infusion and the local health care facility. Coordinate patient referrals from hospitals to home infusion, ensuring smooth transitions and timely service. Participate in quality improvement initiatives and contribute to the development of best practices for home health care. Meet established productivity standards by managing caseloads efficiently document appropriately within designated timeframes. Ensure timely and accurate submission of patient care notes, reports, and other required documentation to meet department and regulatory deadlines. Balance patient care needs with productivity goals, ensuring high-quality care without compromising efficiency. Coordinate with the home infusion pharmacy regarding patient care and insurance coverage. Minimum Requirements Associate's degree in nursing, BSN preferred. Graduate of an accredited nursing program. 1 year of job-related experience. Experience with Central Lines, PICC Lines, Ports, and Peripheral IV’s. Must have valid KY RN license. BLS issued by American Heart Association. Valid driver's license.
