The Care Coordinator will assess the functional abilities of children with disabilities and develop individualized service plans to support their needs. They will also facilitate team meetings, coordinate services with providers, and maintain accurate documentation while ensuring regulatory compliance.
Candidates must hold a bachelor's degree in social work or a related human services field and have at least one year of experience working with children or youth with disabilities. A valid driver's license and proficiency in computer systems and electronic health records are also required.
