The Care Coordinator will assess patient needs, develop individualized care plans, and facilitate communication between patients and multidisciplinary healthcare teams. They are responsible for monitoring patient progress, coordinating appointments, and addressing barriers to care to ensure seamless service delivery.
Candidates must hold a Medical Assistant certification and have at least 2 years of experience in care coordination or case management. Proficiency in electronic health records and strong interpersonal skills are required to effectively support a diverse patient population.
