Position Overview
The Infection Control Preventionist evaluates and revises infection control practices in the long-term care setting to mitigate infections. This role closely monitors the policies, standard work and practices against evidenced based practices and guidelines and provides corrective guidance to clinical and support staff. Provides direct resident assessment of wounds and evaluates treatment regimens in Lake Winona Manor.

Duties & Responsibilities
Tracks and records all communicable infections on an ongoing basis and provides constant feedback to leadership regarding the effectiveness of policies and controls within the service area.
Leads the antibiotic stewardship and vaccination programs within the nursing home for residents.
Collaborates with the Winona Health Infection Control Preventionist and represents Lake Winona Manor by attending and representing Lake Winona Manor at the Winona Health Infection Control Meetings.
Performs routine and random surveillance in the units to identify possible communicable diseases or infections before they can spread.
Leads Vaccination efforts in the facility
Performs and oversees communicable disease as required by state and federal mandates. This includes staff and resident vaccinations, outbreaks, and identification of microorganisms of concern.
Determines the type and duration of isolation precautions when needed.
Reports to the QA&I Meeting all infections by organism on a monthly basis and leads local mitigation efforts on the units.
Provides immediate and ongoing performance feedback/coaching to staff regarding infection prevention and wound care.
Performs weekly wound care assessments and records findings and updates plan of care in collaboration with the nursing team.
Identifies wound concerns based upon observation and record review and reports findings to provider, wound care supplier and nursing team to provide efficient response/action.
Rounds monthly with wound care specialist to review wound healing, progress and goals
Updates individual care plans with goals and interventions for wound care.

Skills and Experience
Required
Active MN RN License
CPR/BLS Certification
3 Years experience as an RN in a clinical setting
Infection Control Certification within 6 months of hire (training will be provided on-site)
Moderate-Excellent Computer skills including Electronic based medical records, Excel, Word, and Sharepoint
Preferred
Prior long term care experience
Experience with evaluating and providing wound care, particularly with staging of ulcerations
Infection Control training
Required Work Schedule
• General work hours are 7am- 3:30 pm, but allows for some flexibility. No weekend or Holiday rotation requirements.
