The Hospice Social Worker conducts psychosocial assessments and develops care plans to support patients and families. They also provide counseling and coordinate community resources while maintaining accurate documentation in the electronic medical record.
Candidates must hold a Master's degree and possess at least one year of healthcare social work experience. A valid Florida license (LCSW, RCSWI, LMHC, RMHCI, LMFT, or RMFTI) and a valid driver's license with reliable transportation are required.
Tidewell Hospice, a part of Empath Health is seeking a Master’s-level Social Worker (MSW) to work with us on a PRN basis and provide assessment, counseling, and support for participants and families. As a key member of the Interdisciplinary Team (IDT), you will help create care plans, connect participants with community resources, and ensure dignity and quality of life for those we serve.
Our PRN shifts are 8 hours per day and are available Mon-Fri.
