Provide individualized, trauma-informed case management and safety planning for survivors of domestic violence, sexual assault, and human trafficking. Maintain accurate client documentation in the EHR system and coordinate wraparound services to support housing stability and self-sufficiency.
Requires a Bachelor’s degree in Human Services or a related field and eligibility as a Mental Health Professional (MHP). Candidates must possess a valid driver's license, maintain First Aid/CPR certification, and pass criminal background checks.

Job Title: Case Manager
Union County Office - Manion (UCOM)
Area of Focus: Behavioral Health
ReLeaf - Is a transitional housing program providing trauma-informed, no-cost housing and wraparound support services for survivors of domestic violence, sexual assault, human trafficking, and community violence across Southern Illinois. This program fills a critical regional gap by offering safe, flexible housing, and individualized case management to help survivors achieve long-term stability.
Program Manager (once hired) In the intrum, Program Director - Daniel Hyson
Full-time 40 hours/ week. Crisis sitter on-call rotation.
FLSA Classification: Non-exempt
Bachelor’s Degree in Human Services or related field, qualifies as a Mental Health Professional (MHP). Previous case management experience is preferred.
Must be 21 years of age or older to drive agency vehicle; maintains a valid driver’s license and vehicle liability insurance; operates agency vehicles and personal vehicles while driving for agency business in a safe and lawful manner at all times; maintain First Aid & CPR certification; must complete required registry clearances and criminal fingerprint background checks; schedules evening or weekend hours, as necessary; participates in crisis sitter on-call rotation.
Knowledge of local health and human services resources. Excellent active listening skills. Skilled in the delivery of case management/care coordination services. Basic crisis intervention and conflict resolution techniques. Excellent written, oral, visual, and non-verbal communication skills. Ability to work independently and collaboratively with team members. Excellent organizational skills. Excellent time management skills and the ability to meet deadlines as assigned.
$44,012 - $46,797 (pay is dependent on education level and credentials)
