The Assessment Specialist conducts in-home visits to assess eligibility and safety needs while developing personalized care plans for individuals. They are responsible for documenting interventions and connecting clients with essential community resources and support services.
Candidates must hold a Bachelor's degree in social work, counseling, psychology, or be a Registered Nurse (RN) or Licensed Social Worker (LSW). A minimum of one to three years of post-degree experience working with chronic conditions or severe mental illness is required.

Possesses a Bachelor's Degree in social work, counseling, psychology, or a related field; or Registered Nurse (RN), or Licensed Social Worker (LSW). Possesses a minimum of one-year post degree experience working with individuals with diagnosed chronic conditions and/or minimum of three years’ post degree experience working with individuals with severe and persistent mental illness. Knowledge of issues affecting people with severe and persistent mental illness and community-based interventions/resources for those individuals. Effective interpersonal (oral and written) skills to interact professionally and ethically with all age groups from a diverse population, including differing intellectual, cultural, ethnic, psychosocial and socio-economic backgrounds. Experience working with individuals with chronic conditions or severe and persistent mental illness Strong knowledge of Medicaid, behavioral health systems, and local community resources.
