The Nurse Practitioner will provide direct patient care in dialysis units and conduct outpatient office visits across multiple locations. They will collaborate with nephrologists to develop treatment plans, manage acute and chronic conditions, and educate patients on their care.
Candidates must hold a Master’s or Doctoral degree in Nursing and possess a current, unrestricted Nurse Practitioner license. National board certification and DEA registration are required, with preference given to those with experience in nephrology or dialysis.

Our board-certified nephrologists and advanced practice providers work collaboratively to support patients with chronic kidney disease, dialysis, hypertension, and related conditions.
Position Overview We are seeking a Nurse Practitioner (NP) to join our team in a position that may be part-time or full-time, depending on the candidate. This role will provide a balanced mix of care in both our dialysis units and office settings, with responsibilities split relatively evenly between the two. The ideal candidate will be a proactive, compassionate professional who values collaboration, continuity of care, and patient education.
· Provide direct patient care in assigned dialysis units, including routine rounds, assessments, and follow-up visits. · Conduct scheduled outpatient office visits across multiple practice locations according to a four-week rotational schedule. · Perform comprehensive histories and physical examinations; assess, diagnose, and manage acute and chronic conditions. · Develop, implement, and monitor individualized treatment plans in collaboration with supervising physicians. · Order, interpret, and evaluate diagnostic tests and laboratory results. · Prescribe medications and therapeutic interventions in accordance with state licensure and collaborative practice agreements. · Document all patient encounters accurately and timely in the electronic medical record (EMR). · Coordinate care with nephrologists, nurses, dietitians, social workers, and other healthcare professionals. · Educate patients and families on disease processes, treatment plans, medications, and lifestyle modifications. · Ensure compliance with all federal, state, and organizational policies and procedures.
· Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program. · Current, unrestricted Nurse Practitioner license in the applicable state. · National board certification as a Nurse Practitioner (ANCC or AANP). · DEA registration and prescriptive authority, or eligibility to obtain. · Prior experience in nephrology, dialysis, internal medicine, or primary care preferred. · Proficiency with electronic medical record systems. · Strong organizational, communication, and clinical assessment skills.
