The Social Worker will develop individualized care plans and social histories to support the emotional and medical needs of residents. They will also coordinate discharge planning and facilitate communication between residents, families, and external healthcare partners.
Candidates must hold a Bachelor’s Degree in Social Work or a relevant certification. Previous experience in a long-term care setting is required along with strong interpersonal and organizational skills.
