The Qualified Professional will provide direct support, skill-building services, and therapeutic interventions to youth with behavioral and mental health needs in home and community settings. Responsibilities include managing caseloads, maintaining clinical records, and facilitating person-centered planning to ensure client stability and success.
Candidates must possess a bachelor's degree in a human services field and at least two years of documented experience working with youth or adolescents with mental health needs. Strong organizational skills, reliable transportation, and the ability to adhere to state service definitions are required.
Qualified Professional (IIH) Key Responsibilities Complete initial intakes, Person‑Centered Plans, and submit authorizations/reauthorizations Maintain accurate, up‑to‑date clinical records, including consents, intake packets, and required documentation Complete and submit NC TOPPS, pre/post outcomes, discharge SARs, and other program paperwork Coordinate care, scheduling, and person‑centered planning for assigned clients Assist in identifying placements for clients and completing required placement documentation Monitor caseloads, track authorizations, and verify insurance monthly Provide strength‑based, recovery‑focused interventions Support skill building, coping strategies, and community functioning Perform other duties as required Qualifications Bachelor’s degree in a human services field (required) Two years of documented experience working with youth/adolescents with mental health needs Strong documentation and organizational skills, including ability to maintain logs and clinical records Ability to teach skill‑building activities and support clients in home and community settings Reliable transportation, valid driver’s license, and active auto insurance Knowledge, Skills, and Abilities Ability to engage youth and families in a supportive, therapeutic manner Strong communication skills and the ability to collaborate with diverse populations Ability to follow state service definitions and program requirements Ethical, professional, and reliable work habits Strong time‑management and problem‑solving skills Work Environment Home and community‑based service delivery Travel required within Person, Alamance, and Caswell counties Some remote tasks may be available based on program needs
