The Care Manager RN coordinates discharge planning, transitions of care, and case management activities to ensure optimal patient outcomes. They collaborate with interdisciplinary teams and review medical records to ensure compliance with federal, state, and accreditation standards.
Candidates must have 2-4 years of clinical nursing experience in a hospital, home health, or nursing home setting. A Bachelor's degree in Nursing is preferred, and a valid RN license is required.
