Provide comprehensive medication management for chronic disease states and manage anticoagulation therapy in collaboration with the healthcare team. Serve as a key member of the care team to provide drug information, therapeutic recommendations, and clinical documentation within the electronic health record.
Requires a Doctor of Pharmacy (PharmD) degree and completion of a PGY1 Pharmacy Practice Residency. Candidates must hold a current Michigan pharmacist license and possess certification in Anticoagulation Management.
Conduct patient assessments, develop care plans, and monitor therapeutic outcomes in collaboration with the healthcare team. Responsibilities Manage anticoagulation therapy (e.g., warfarin, DOACs) including patient education, INR monitoring, dose adjustments, and documentation. Participate in transitions of care to reduce hospital readmissions and improve continuity of care. Complete comprehensive medication reviews as required. Qualifications Required: Doctor of Pharmacy (PharmD) degree from an ACPE-accredited institution. Completion of a PGY1 Pharmacy Practice Residency (PGY2 in ambulatory care preferred). Current pharmacist license in Michigan. Certification in Anticoagulation Management or equivalent experience. Strong clinical knowledge and communication skills. Proficiency with EHR systems and clinical documentation. Required Skills Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS). Experience working with and utilizing a collaborative practice agreement setting. Preferred Skills Outpatient clinics and physician offices. Anticoagulation clinic setting. Occasional travel between clinic sites may be required.
