The Clinical Pharmacist is responsible for optimizing patient care in infectious diseases by identifying, resolving, and preventing drug-related problems. They also participate in clinical committees, develop drug guidelines, and provide educational support to healthcare staff and students.
Candidates must hold a Pharm.D. degree from an ACPE-accredited school and have completed a PGY2 pharmacy residency in infectious diseases. A valid Registered Pharmacist license is also required for this position.
A day in the life of a Clinical Pharmacist, Infectious Diseases at Hackensack Meridian Health includes: 1. Identifies, resolves, and prevents drug-related problems.2. Recommends and monitors drug therapy.3. Participates in the department`s drug formulary review process, including the preparation of drug monographs for review by the Pharmacy and Therapeutics Committee.4. Participates in drug guideline and order set development.5. Participates in drug research-related activities.6. Participation on various Medical Center committees and service lines as dictated by specific area(s) of expertise (e.g., cardiology, oncology, pediatrics).7. Provides drug information to various health care providers.8. Serves as a resource to the department in area(s) of expertise by helping to ensure that drug treatment plans and drug therapies are safe and effective.9. Participates in and assists in conducting educational programs for physicians, nurses, pharmacists, pharmacy students, and other health care providers.10. Participation in pharmacy student rotation program.11. Participates in the Pharmacy Practice Residency program as a preceptor and member of the Residency Advisory Committee.12. Participates in the Antimicrobial Stewardship Program, the Pharmacy and Therapeutics Committeeand the Antimicrobial Utilization Sub-committee.13. Assists with special projects as required.14. Assists with medical use evaluations.15. Assists with staffing as deemed necessary by the Director of Pharmacy.16. Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing, and/or visually impaired, etc.) This process includescommunicating with the patient, parent, and/or primary caregiver(s) at their level (developmental/age,educational, literacy, etc.).17. Other duties and/or projects as assigned.18. Adheres to HMH Organizational competencies and standards of behavior.
Qualifications: Education,
Required: 1. Graduate of an ACPE-accredited school of pharmacy with a minimum of a Pharm.D. degree.2. Completion of a clinical pharmacy residency program with two years of pharmacy residency training PGY2 in Infectious Diseases.3. Excellent written and verbal communication skills.4. Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms. Licenses and Certifications Required: 1. Registered Pharmacist License.
