Provides psychosocial evaluations, psychotherapeutic interventions, and case management services for patients and their families. Acts as a liaison between the hospital, community agencies, and physicians while facilitating treatment plans and discharge services.
Requires a Master's degree in Social Work, Counseling, Psychology, or a related field. Candidates must possess at least two years of post-graduate behavioral health experience and hold a valid clinical license such as LMSW, LISW-CP, LISW-AP, LPC, or LPCA.
