The Physical Therapist will assess and implement care plans for patients in a home setting while evaluating their progress. They will also collaborate with an interdisciplinary team and provide instruction to students, aides, and volunteers.
Candidates must be licensed or eligible for licensure in the State of Virginia and possess a valid driver's license. One year of clinical or home health experience is preferred, along with strong communication and time management skills.
Augusta Health is seeking a part time Physical Therapist to join the Augusta Health team that is making a difference in the lives of our patients in the Shenandoah Valley by providing quality in-home care.
The Physical Therapist for Home Health will assess and implement plan of care needs to patients in a home setting and provide treatments to patients and evaluates progress as well as assist in the instruction of additional physical therapy personnel, students, home health aides, and volunteers. The individual must demonstrate the ability to interact with team members for an interdisciplinary team approach. Patient care is provided across the life span; therefore, clinicians will interpret data and assess patient’s needs relative to age specificity standards.
