Grant summary

The goal of the Hope Project is to improve mental health outcomes for children and youth at risk for or with serious emotional disturbance (SED) and to bring new hope through its comprehensive and early intervention approach for the children, youth, and their families.

Position summary

This position is responsible for providing case management services for project family participants (consumers). The position orients families to the Hope Project which leads the families to attend monthly individual meetings and facilitated group quarterly meetings.

• Job duties
• Facilitates comprehensive orientation for designated consumer family members, as needed.
• Facilitates quarterly family gatherings.
• Meets with designated consumer family members monthly.
• Promotes the Health and Wellness of the family by covering physical, emotional, intellectual, social, environmental, occupational, spiritual, land-centric, and financial education.
• Works with families to understand and address risk factors and connects consumers with resources and support services.
• Collaborates with FNA BHS child, youth, and young adult services.
• Participates in continuous quality improvement and data-driven decision-making processes to assess program efficacy and drive quality improvement.
• Ensures adherence to data collection requirements and enters accurate and timely data into the Electronic Health Record (EHR).
• Develop billing records.
• Provides culturally relevant and trauma-informed service.
• Attends mandatory position training that provides training in risk factors as designated by Project Director and/or Clinical Supervisor.
• Attend all required staff meetings.
• Performs other job-related duties as assigned.

• Necessary knowledge, skills, and ability
• Ability to report to work on time.
• Knowledge of substance abuse and mental health; and appropriate skills in working with consumers with Mental Health/Substance Use disorders.
• Skill in working successfully with the Alaska Native/American Indian population and experience providing culturally specific programming; understanding of and sensitivity to diverse cultures and lifestyles.
