The Registered Nurse will deliver direct nursing care, including triage, wound care, and blood draws, while collaborating with an interdisciplinary team to manage patient care plans. The role involves performing assessments across clinic, day center, and home settings, as well as participating in an after-hours on-call rotation.
Candidates must hold a valid Registered Nurse license and obtain BLS certification within 30 days of hire. A diploma from an accredited nursing school is required, with at least one year of RN experience preferred.
$31.91 - $45.05 per hour

How you’ll make an impact in this role Deliver high-quality, direct nursing care to PACE participants, including acute triage, blood draws, wound care, and participation in an after-hours/weekend on-call rotation. Collaborate with the Interdisciplinary Team (IDT) to develop, implement, and evaluate individualized care plans, ensuring timely updates based on participant needs. Participate in IDT care plan and participant/family meetings, and perform assessments across care settings such as the PACE clinic, Adult Day Center, and participant residences. Educate participants, caregivers, and staff on health management and end-of-life care, with flexibility to travel to homes, long-term care, and assisted living facilities as needed.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
What additional preferences we're seeking One or more years of RN experience. Home Health experience a plus.
