Requires a degree in social work (LMSW/LBSW) or a related field with supervised experience in a healthcare setting. Must possess a valid driver's license and the ability to travel to patient homes.

Assists in the admission process of the client to the Agency by performing an initial evaluation, assessing the client’s psychosocial, financial, environmental and community needs as evidenced by POC, documentation, clinical records, conferences, and community resource referrals. Demonstrate commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards and requirements. Collaborate with interdisciplinary team to promote coordination of patient care. Accept clinical assignments that are consistent with education and competence to meet the needs of the patients. Provides effective communication to clients, their families, team members, and other health care professionals to achieve desirable outcomes, and regular participation in Case Conference/Team Meetings. Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency. Meet mandatory continuing education requirements of the Agency/licensing board.
Education and experience: Education: Social Work (LMSW/LBSW) degree from a school of social work accredited by the Council on Social Work Education; or Baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or Baccalaureate degree in psychology, sociology, or other field related to social work and is supervised by an MSW; and has 1 (one) year of social work experience in a healthcare setting; or Baccalaureate degree from a school of social work accredited by the Council on Social Work Education, is employed by the Hospice before December 2, 2008 and is not required to be supervised by an MSW.
