The Licensed Clinical Social Worker will provide individual, family, and group therapy while conducting clinical assessments and treatment planning for residents in long-term care facilities. They will also collaborate with interdisciplinary teams to coordinate care and maintain accurate clinical documentation.
Candidates must hold an active, unrestricted LCSW license and possess a valid driver's license with reliable transportation. Strong interpersonal communication skills and the ability to work effectively within an interdisciplinary team are essential for this role.
