Status:
Full-time, permanent position (40 hours)
Standard working hours: 8am to 5pm EST, M-F.

Work with Sentara Health Plan (SHP) members to close medication-related gaps in care Work on the referrals from the team Counseling members on their medication therapy Education BLD - Bachelor's Level Degree in Pharmacy REQUIRED Certification/Licenses Pharmacist License (RPh) - State license - Other/National REQUIRED Will consider new graduated pharmacists and pharmacists not licensed in Virginia due to relocation. Must have Virginia license within 90 days of Sentara start date.
