The Registered Nurse will conduct nursing assessments and develop individualized service plans for individuals with developmental disabilities in home and community settings. They are also responsible for providing skilled nursing care, including tube management and port maintenance, while ensuring compliance with state regulations.
Candidates must hold an active, unencumbered Texas RN license and have at least one year of experience in home health, hospice, or community-based settings. Proficiency with mediports, reliable transportation, and current CPR certification are also required.
American Medical Home Health & Hospice Services is seeking an experienced and compassionate Registered Nurse (RN) to join our Community Living Assistance and Support Services (CLASS) Program team serving Corpus Christi and nearby communities. The RN will provide professional nursing services to individuals with developmental disabilities or related conditions who receive in-home and community-based care. This role requires strong clinical judgment, and familiarity with skilled nursing services.
Conduct initial and ongoing nursing assessments, identifying the medical, functional, and psychosocial needs of CLASS participants. Develop, implement, and regularly update Individualized Service Plans (ISPs) in coordination with the case manager and interdisciplinary team. Provide skilled nursing services such as G-tube and J-tube management, and Medi port access, flushing, and maintenance. Educate participants, family members, and attendants on proper medication administration, infection control, and health maintenance. Collaborate closely with physicians, therapists, and direct care staff to ensure coordinated and quality service delivery. Document all assessments, skilled nursing visits, and communications in compliance with state regulations and agency policies. Ensure all nursing care provided aligns with the Texas Health and Human Services CLASS Program guidelines and standards. Conduct periodic supervisory visits to evaluate attendant and participant compliance with care plans.
Active Texas RN License (unencumbered, in good standing). Minimum of 1 year of nursing experience in home health, hospice, or community-based settings. Experience and proficiency with mediports required. Strong knowledge of CLASS program requirements and waiver service delivery preferred. Exceptional communication, assessment, and documentation skills. Reliable transportation, valid driver’s license, and proof of auto insurance. Ability to travel throughout the Corpus Christi area and surrounding counties. Bilingual (English/Spanish) preferred. CPR Certification (current). Strong critical thinking and problem-solving abilities.
