The Physical Therapist will perform evaluations and develop individualized treatment plans to improve patient mobility and independence. They are responsible for providing skilled therapy services in home settings and documenting progress in the EMR system.
Candidates must hold a current Texas Physical Therapist license and possess a valid driver's license with reliable transportation. Home health experience and bilingual English/Spanish skills are preferred.
American Medical Home Health is seeking a dedicated and compassionate Physical Therapist (PT) to provide quality home health therapy services to patients in the west and southwest areas of San Antonio, including surrounding communities and outskirts beyond Loop 1604.
We are looking for a therapist who is dependable, patient-centered, and comfortable working independently in the field while collaborating closely with our clinical team.
The Physical Therapist will evaluate patients, develop individualized therapy plans of care, provide skilled therapy services, and help patients improve mobility, strength, safety, and independence in the home setting. Service Area
West San Antonio, Southwest San Antonio, and surrounding/outskirt areas past Loop 1604, including nearby rural or semi-rural areas as assigned.
Perform initial and ongoing physical therapy evaluations. Develop and update individualized therapy treatment plans. Provide skilled PT services in accordance with physician orders and the patient’s plan of care. Educate patients and caregivers on exercises, safety, fall prevention, transfers, mobility, and home programs. Document visits accurately and timely in the EMR system. Communicate patient progress, concerns, and changes in condition to the clinical team. Participate in care coordination with nursing, therapy, and administrative staff. Follow all Medicare, state, agency, and professional standards.
Current Texas Physical Therapist license in good standing. Home health experience preferred Strong clinical assessment and communication skills. Reliable transportation and valid driver’s license. Ability to manage a field schedule independently. Comfortable traveling throughout the assigned service area. Bilingual English/Spanish preferred but not required.
