The Hospice Nurse is responsible for providing professional nursing care utilizing the nursing process, which includes assessing patient status, developing and revising plans of care, and administering prescribed medications and treatments. This role involves communicating with physicians and community health persons to coordinate care and ensuring all patient needs are met promptly.
Applicants must be graduates of an accredited school of Nursing and hold an active RN license in the state of care provision. While one or more years of experience in Hospice, home health, or a related organization is preferred, it is not strictly required.

As a Transitions Care Hospice Nurse you are responsible for providing care to patients in need. The Nurse functions as a support team member to the IDG team by providing additional services when required. The Nurse will assess, address and report patient needs to the case management team all while ensuring that all patient needs are met in a timely manner. Local travel is required.
Education and Experience Graduate of an accredited school of Nursing Nursing licensure in the State which you provide care (RN). One (1) + year Hospice, home health or related organization experience preferred.
