The Hospice Nurse is responsible for providing professional nursing care utilizing the nursing process, which includes assessing patient status, writing and revising the plan of care, and administering prescribed medications and treatments. Key duties involve completing all required visit types, maintaining accurate clinical notes, and communicating patient needs and changes to the physician and case management team.
Applicants must be graduates of an accredited school of Nursing and hold an active RN license in the state where care is provided. While one or more years of experience in Hospice, home health, or a related organization is preferred, it is not strictly required.

As a Transitions Care Hospice Nurse you are responsible for providing care to patients in need. The Nurse functions as a support team member to the IDG team by providing additional services when required. The Nurse will assess, address and report patient needs to the case management team all while ensuring that all patient needs are met in a timely manner. Local travel is required.
Education and Experience Graduate of an accredited school of Nursing Nursing licensure in the State which you provide care (RN). One (1) + year Hospice, home health or related organization experience preferred.
