The Case Manager will conduct intake interviews, develop intervention plans, and coordinate community resources to support clients in recovery. They will also provide crisis intervention and facilitate discharge planning while collaborating with a multidisciplinary team.
An Associate's degree is preferred, though equivalent knowledge and experience in substance abuse and community support services will be considered. A valid and unrestricted Driver's License is required to operate an agency vehicle.

We are seeking a compassionate and resourceful Case Manager to provide client placement, referral, and discharge planning services — connecting individuals in recovery with the resources, support, and pathways they need to build meaningful, lasting lives.
As a Case Manager, you will serve as a vital guide and advocate for clients navigating the recovery process — conducting intake interviews, developing intervention plans, coordinating community resources, and providing crisis support. You will work collaboratively across disciplines to ensure every client has access to a continuum of care tailored to their individual needs, goals, and strengths.
