The Hospice Social Worker provides medical social services, psychosocial support, and bereavement counseling to patients and their families. The role requires commuting to various territories across Brooklyn, Queens, Nassau, and Manhattan to ensure continuity of care.
Candidates must hold an MSW from an accredited school and be eligible for a New York LMSW or LCSW license. A valid driver's license and a personal vehicle are required for travel to patient locations.

Minimum one (1) year bereavement counseling experience is preferred. At least one year of Hospice or Home Care experience preferred. Excellent interpersonal, and communication skills. Position requires ability to commute to multiple territories to ensure continuity of care. A valid driver’s license is required. A vehicle is required to commute to patients. (This requirement does not apply to positions assigned exclusively to Manhattan territory) Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.
