Job Responsibilities
Follows the seven rights of medication administration while reporting adverse reactions to the physician immediately and taking action in emergency cases.
Coordinates referrals for patient chronic care conditions and follow up care as indicated by the initial care plan of the specialist and/or primary care physician.

Administers prescribed oral and intramuscular medications, therapies and treatments following practical nursing standards and under the direct supervision of the physician.
Effectively communicates with healthcare team members, patients and families; Supports healthcare team members to achieve team goals and assigns appropriate tasks to care assistants.
Maintains and reviews patient medical records, notes and physician orders; Documents all patient care activities in accordance with documentation standards and legal requirements for medical facilities.
Upholds the standards of Patient-Centered Medical Home (PCMH) and assists other clinical staff members in completing and following quality measures under PCMH and Patient-Centered Specialty Practice (PCSP).
Creates a plan of care that is patient-centered and provides a written copy of the plan of care to patients.
Provides direct patient care, utilizing the nursing process, under the direction and supervision of the physician.
May perform additional duties as assigned.
Requirements
Certification - Basic Life Support
Licensure - Licensed Practical Nurse licensed or eligible for licensing in the state of employment as a Licensed Practical Nurse
Experience - 2 years of nursing practice in a hospital or clinic setting
Education - Vocational/Technical Graduate of an accredited Licensed Practical Nurse school

Preferred experience: clinical experience in outpatient medical office setting
