The Case Manager provides clinical and case management services, including resource referrals, counseling, and crisis intervention for unhoused adults. They are responsible for maintaining accurate client documentation, conducting field work, and facilitating client groups.
A bachelor's degree is required, with a preference for social work or related fields. Candidates should have at least one year of case management or counseling experience and knowledge of NYC housing subsidy programs.
The salary range is based on several factors, including, but not limited to, overall experience, relevant experience, education level, certifications, applicable skills, and expertise.
Hybrid: 4 days in office/1 day remote

Reporting to the Director of Social Services/Social Services Supervisor, the Case Manager provides and documents clinical and case management services for assigned clients. The Case Manager will be an active listener and provide high quality customer service and respond by exhibiting cross-cultural awareness with the ability to promote and contribute to an equitable and inclusive anti-racist organizational culture and environment.
1. Provides comprehensive case management services for assigned clients including, but not limited to:
1. Support and resources/referrals for clients to meet objectives, including but not limited to:
1. Entitlements
2. Housing
3. Employment / Education / Vocational Training
9. Health, mental health treatment, and substance abuse services
22. Social and community integration
2. Counseling and support to clients on an on-going basis including, but not limited to:
1. Crisis intervention
2. Training and support on client daily living skills, as required
3. Assistance in problem solving, as required
9. Conflict resolution
22. Advocacy
2. Assesses client needs, develops service plans and/or service referrals to achieve client self-sufficiency.
3. Conducts field work including, but not limited to home visits, escorting clients to appointments, and outreach.
4. Facilitates client groups.
5. Monitors client medications, as required.
6. Conducts client intake and updates information in client database system(s).
7. Completes incident reports and distributes related notices within 24 hours.
1. Maintains and submits all necessary documentation and reports accurately in a timely manner and as required by Urban Pathways, regulatory and funding agency policies, and procedures including but not limited to:
1. Enter progress notes after each client meeting within 48 hours and/or as mandated by the Program.
2. Completes Intake/Admission documentation within 24 hours of client admission, as mandated by the Program.
3. Documents assessments in client records within required timeframe (not applicable for Outreach Program).
4. Updates information in client records, as required.
5. Documents incidents and distributes related notices within 24 hours when the Case Manager has witnessed the incident.
6. Submit all required paperwork for supervisor approval within 48 hours.
7. Files paperwork in client charts (once approved/signed by supervisor), as required by program (no more than 48 hours).
1. Develops and maintains relationships with community services and resources with particular emphasis on substance/alcohol abuse programs, housing, health and mental health programs, employment, and benefits.
2. Attends and participates in external meetings with funding agency and housing providers.
1. Communicates verbally and in writing in a professional manner at all times.
2. Attends staff meetings, trainings, and supervisory sessions, as required.
3. Responds to all communications including telephone, email, and other oral and written inquiries in a timely manner.
Performs all other duties as assigned and as required.
