The Hospital Liaison acts as a communication hub and coordinator for clients throughout the psychiatric hospitalization process, from intake to aftercare. They are responsible for tracking cases, ensuring continuity of care, and maintaining relationships with hospitals and emergency departments.
The position requires a High School Diploma or GED. Candidates must be organized professionals capable of managing complex communication between families, clinical staff, and external providers.
Description Astor is seeking an organized professional to build and maintain relationships with hospitals and emergency room departments in the Bronx and the Hudson Valley regions. The liaison will serve as both the communication hub, internal coordinator, and a supportive family contact throughout the hospitalization process, from intake, through inpatient care, discharge and aftercare/return to Astor treatment, ensuring continuity of care of all client psychiatric hospitalizations.
Maintain responsibility for actively tracking, following, and maintaining cases through the entire hospitalization process, including referral, admission, treatment, discharge, and post-discharge. Ensure that family/client perspective and expertise are prioritized throughout the hospitalization process. Work with program leadership to develop policies and procedures. Organize communicating among clients, families and caregivers, collateral providers, and various levels of internal and external peer, clinical, medical, and leadership staff. Utilize the agency electronic health record to document contacts consistent with programmatic and agency expectations and regulations. Follow HIPAA practices, and collaborate with quality assurance to meet best practices markers and goals. Attend relevant treatment team meetings, clinical incident command, and/or Quality Assurance/Improvement and Incident Review calls relevant to care coordination and/or process improvement. Work with Astor’s director of clinical outcomes and data management team related to key performance indicators and other agency objectives regarding quality of care throughout course of hospitalization. All work to be conducted in line with the agency’s standards of care and core values, driven by the mission and vision of the agency. This concierge-like relationship will be person-centered, family-driven, clinically-informed, and articulate client, program, and agency needs to improve quality and continuity of care.
