Generations at Applewood in Matteson, IL, has an immediate opening for a MDS Nurse (RN or LPN) Salary is $90k-95k depending if a RN, MDS Certification and experience. Salary for a LPN is $79k-$85k also depending on MDS Certification and experience.

Duties

of the MDS Nurse: The MDS Coordinator is responsible for collaborating with the Interdisciplinary Team Plan, organize, develop and implement the MDS assessments and care plans Update records and assessments, ensuring information is consistent with patient care plans and that scoring is accurate Accountable for accurate MDS scoring Generations at Applewood is a 24-hour rehabilitation and skilled nursing community offering quality healthcare services to senior residents throughout the Chicagoland area. Requirements: of the MDS Nurse:

Current RN or LPN licensed from the State of Illinois American Association of Nurse Assessment Coordination (AANAC) certification preferred RAC-T Certified preferred Knowledge of Minimum Data Set (MDS), ICD-10 Coding Two (2) to three (3) years of applicable experience as a MDS Nurse Must be CPR Certified Experience in a long term care setting Generations at Applewood is an Equal Opportunity Employer
