The nurse practitioner will provide a full range of medical services including diagnosis, treatment planning, and specialty referrals for patients of all ages. They will also serve as an active member of the Patient Centered Medical Home team to ensure continuity of care.
Candidates must hold an active, unrestricted medical license and possess at least three years of experience in treating both adult and pediatric patients. Required certifications include BLS, ACLS, PALS, and ATLS, along with the ability to pass a federal security clearance.

1. Provide a full range of medical services including evaluation, diagnosis, consultation & referral, specialty referrals for outpatients of all ages. 2. Serve on the Patient Centered Medical Home team. 3. Makes referral recommendations to the Clinical Director as required ensuring appropriate specialty care, follow-up, continuity of care and ensuring appropriate forms are completed. 4. Participates in treatment planning with the medical department and for individual patients.

Active, Unrestricted Medical License: Must hold a current and unrestricted license to practice as a Nurse Practitioner in any U.S. state, the District of Columbia, or the Commonwealth of Puerto Rico.
