The Discharge Planner functions as part of a multi-disciplinary team to plan patient treatment and continuing care. They assist patients and families in selecting and utilizing appropriate healthcare services.
Candidates must be fluent in medical terminology and possess the ability to interpret technical documents and procedure manuals. A BLS certification is required for this position.
Overview Functions as part of the multi-disciplinary team in planning appropriate treatment and continuing care plan for patient.
Plays an integral part in discharge planning of the patient and assists the patient/family in choosing/utilizing appropriate services. Responds to referrals.
Fluent in medical terminology. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. BLS Certification.
