The role involves evaluating patients of all ages, establishing individualized treatment plans based on functional goals, and implementing/modifying these programs to support long-term outcomes. Responsibilities also include documenting clinical findings, providing patient/family education, and collaborating with the care team.
Candidates must be graduates of an accredited Physical Therapy program and possess a current or pending New Hampshire Physical Therapy license, along with CPR Certification. Experience in outpatient Physical Therapy (1–2 years) is preferred, along with strong skills in time management, communication, and documentation.

Our outpatient Physical Therapists work with advanced equipment, experienced clinicians, and strong interdisciplinary support to deliver exceptional care. About the Job Provides physical therapy services to outpatients and/or inpatients of all ages—from adolescents to adults and geriatrics—with occasional developmentally based exceptions depending on clinical need. What You’ll Do Evaluate patients using subjective data, objective measures, clinical assessment, prognosis, and function‑based goals, establishing effective and individualized treatment plans. Implement and modify patient‑specific treatment programs to support functional progress and long‑term outcomes. Develop and deliver patient and family education, adjusting strategies as needed to maximize engagement and understanding. Document all clinical findings, plans, and progress in the electronic medical record in a timely and accurate manner. Complete all required insurance and third‑party payer information. Provide clinical supervision to new staff members and/or affiliating students as assigned. Collaborate with physicians, care teams, and support staff to ensure coordinated and effective treatment. Maintain clinical competency and utilize equipment and techniques appropriate to training and experience. Demonstrate excellent customer service and support a safe and positive patient experience. Who You Are Graduate of an accredited Physical Therapy program. Current (or pending) New Hampshire Physical Therapy license required. CPR Certification. Experience in outpatient PT preferred (1–2 years). Graston Technique Lymphedema Certification Dry Needling Strong time‑management, communication, and documentation skills. Ability to provide safe patient handling and demonstrate appropriate physical and clinical judgment.
