The Social Worker performs psychosocial assessments of patients and families to develop and implement effective care interventions. They also provide psychoeducation and supportive counseling while collaborating with the interdisciplinary group to improve patient outcomes.
The role requires a professional social worker capable of providing expert psychosocial support within a hospice setting. Candidates must demonstrate strong teamwork, professionalism, and a commitment to end-of-life care excellence.
