Requires an active Georgia RN license, BLS/CPR certification, and at least one year of clinical experience in infusion or IV therapy. A valid driver's license and reliable transportation are mandatory for travel to patient sites.
Home Infusion Nurse - Partner With Patients. Practice With Purpose. As a Home Infusion Nurse, you’ll provide one on one, skilled clinical care to patients where they feel most comfortable - at home or in an ambulatory infusion suite. This role offers true autonomy, a flexible schedule, and the opportunity to build meaningful connections with patients and caregivers while delivering life changing therapies.
You’ll have the support of a multidisciplinary care team, comprehensive training, and the resources you need to deliver safe, effective, and compassionate infusion care.
Travel to patient homes and related sites using a personal or rental vehicle in accordance with company travel policies Provide safe and compliant infusion services in accordance with Optum Infusion Pharmacy policies, protocols, and physician orders Establish and maintain positive therapeutic relationships with patients and caregivers, including education on self-care and self-administration Assess patient suitability for home infusion therapy, including home safety, caregiver support, and patient independence Obtain and maintain accurate patient medical histories, clinical assessments, and all required documentation, including plans of care, visit notes, medication profiles, and discharge summaries Deliver skilled nursing care across a range of patient ages and conditions based on physician orders and treatment plans Establish, access, and maintain vascular devices, including peripheral IVs, PICC lines, tunneled catheters, and implanted ports Administer medications and monitor patient responses, including managing infusion reactions, vital signs, side effects, and lab collection Communicate patient status and therapy response promptly with physicians, pharmacists, supervisors, and other care team members Assist in staff orientation and training, serving as a clinical educator and resource as requested

Active and unrestricted Registered Nurse (RN) license in the state of Georgia Current BLS/CPR certification or willingness to obtain prior to start 1+ years of RN experience in a clinical setting supporting infusion or IV therapy (e.g., ICU, Med Surg, ED, PACU) Proven ability to work independently with solid clinical judgment and effective interdisciplinary collaboration skills Valid driver’s license and reliable transportation
Home infusion experience Knowledge of Infusion Nursing Society (INS) standards Familiarity with Microsoft Office and EMR platforms Understanding of HIPAA, OSHA, and home infusion accreditation guidelines Proven solid communication, organization, critical thinking, and teaching skills
