Provide one-to-one compassionate paraprofessional care to patients in their homes following RN care plans. Responsibilities include assisting with personal hygiene, household tasks, monitoring vital signs, and reporting condition changes to the nursing staff.
Must be a Certified Nursing Assistant (CNA) with a high school diploma or GED and completion of a 75-hour course. Requires one year of experience as an aide in an institutional, home care, or hospice setting, along with a valid driver's license.

Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program. Documentation of one year’s employment as an aide in an institution, home care, or hospice setting. Meets agency hiring criteria as defined in agency minimum standards (e.g. provision of professional references.) Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency’s client and their family. Must accept responsibility for maintaining skills, learning and adhering to agencies policies and procedures on an on-going basis. Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics, and adhere to the Compliance Program.
