Shift: Day Initial offers are made at the hourly base rate of $19. Additional compensation may be available after hire based on factors such as driving responsibilities, required certifications (e.g., AMAP), and length of time in the role. Pay increases within the range are based on certifications, training completion, and job performance. Hourly rate is only one part of Heartshare’s total compensation package, which may also include a bi-annual discretionary bonus. The Day Hab Support Professional is responsible for implementing person-centered services for each program participant. This includes ensuring implementation of the individual’s goals/valued outcomes in relation to both center-based and community inclusion activities. In addition, the Day Hab Support Professional is directly responsible for the individual program participant’s overall safety and well-being.

Professional will: Implement individual program participant’s Day Services Plan, consistent with their specific goals and valued outcomes. Participate in the planning and implementation of activities, both at the program center and in the community, that address these goals and outcomes. Train/teach program participants according to their individual goals and valued outcomes. Facilitate skills development, socialization and active participation in conjunction with group room and community activities. Ensure that support is provided with consideration for each person’s individualized needs and routines. Facilitate inclusion of program participants in their community, according to their individual needs, interests, and desires. Support program participants in forming and maintaining friendships with neighbors, co-workers and peers. Encourage interaction between individual program participants and community members. Remain focused on facilitating the development of personal relationships for individual program participants. Transport program participants in agency vehicles, as required, for community inclusion, recreational purposes and to and from their home. Participate in planning group recreational trips and outings. Participate as requested in the Interdisciplinary Team meetings for program participants for whom direct support and services are provided. Assist in timely collection of consumer-related information, including behavioral data, goal-related data, community inclusion logs, communication books, etc., ensuring their accuracy and completeness. Complete all written reports and documentation on a timely basis. Ensure that program participants are clean and safe at all times. Ensure that no program participant is inappropriately left alone or unsupervised. Participate in bussing, lunchtime meals, and toileting of program participants, as assigned. Accompany program participants to clinic and/or emergency room visits as assigned. Provide crisis intervention and counseling as required. Report all incidents, including allegations of abuse and neglect and serious reportable incidents, to program management immediately upon discovery. Ensure that group room equipment and supplies are appropriately and safely stored. Assist in ensuring overall program orderliness and cleanliness, particularly in assigned group room. Report any hazards or unsafe conditions immediately to the supervisor. Undertake any additional tasks and initiatives within the scope of the role as per the specific needs of the programs and individuals and as assigned by supervisor. Qualifications High school diploma or GED required. CNA (Certified Nursing Assistant certification) or possess a Direct Support Professional (DSP) certificate from an accredited public or private organization preferred. One or more years' experience working with individuals with developmental disabilities preferred. Valid NYS driver's license preferred but not required.
